Summary: Background: The abuse of drugs is often related to violence, but there are no studies show quantitative toxicological analysis in fatal victims in the Metropolitan Region of Vitória – Espírito Santo. Aims: To identify and quantify alcohol, cocaine and its metabolits and crack cocaine through its pyrolysis product in homicide victms in the metropolitan region of Vitória, Espírito Santo. Methods: This is a cross-sectional in single-center study examining the presence and blood concentrations of alcohol, cocaine and benzoylecgonine and anhydroecgonine methyl ester , as indicative of crack use, in a sample of 485 out of 976 homicide victims in the metropolitan region region of Vitória – ES during the
year 2014. Result: The great majority (85.3%) was young victims aging less than 18 to 40 years. A high proportion (48.7%) showed either alcohol alone (25.8%) or crack or cocaine alone (41.1%), or a mixture of cocaine and alcohol (33.1%). A high percentage of crack or cocaine was found in the extreme age groups: the youngest, less than18 years (73.3%) and 18 to 30 years (52.7%), and the oldest, 51 to 60 years (50%) and above 60 years (66.7%). Although at a smaller number (9.1%), females victims were mostly positive for drugs (54.5%), 50% of which for crack or cocaine. Greater amounts of alcohol were found in adult and old in victims; cocaine in young adults and anhydroecgonine methyl ester among the youngest benzoylecgonine was found
in higher amounts in women. Conclusion: Almost half of the sample of homicide victims from an metropolitan region of Vitória, Espírito Santo showed drugs in their blood. Some of them, especially very young or females, with very high blood concentrations of cocaine, benzoylecgonine or anhydroecgonine methyl ester.
